With the required pre-approval from the ODVA Conformance Authority, you may request that ODVA Amend and Restate a previously-issued DOC based on changes in the Minor Revision of the products listed on the DOC.
The form below is for use ONLY to request that ODVA Amend and Restate an ODVA Declaration of Conformity (DOC), which is currently valid for products listed on the DOC, for a change in the Minor Revision of the product, as defined by the CIP Identity Object, Attribute 4, which has zero impact to the CIP and network functionality in the device and as allowed in ODVA’s Policy Regarding Compliant Products.
