ODVA 2022 Industry Conference & 21st Annual Meeting of Members (San Diego, Calif., USA)

Together with the Hyatt Regency Mission Bay, ODVA Inc. (ODVA) has taken preventative measures to reduce the spread of Communicable Disease including COVID-19 (“Communicable Disease”) at its 2022 Industry Conference & 21st Annual Meeting (the “Event”). However, I recognize that even diligent efforts to follow public health guidance and orders cannot guarantee that attendees will not contract or transmit the virus that causes COVID-19 or other Communicable Diseases, even if the participants have been vaccinated against COVID-19, and that participants or those they encounter face risks of infection from this potentially deadly disease as a result of traveling to and participating in the Event and related activities. ODVA will continue to monitor the ongoing COVID-19 situation, the CDC’s recommendations, and federal, state, and local requirements. Should any changes to the protocols be required prior to the Event, an updated protocol will be provided to all Event attendees.

As a participant in the upcoming Event, I agree that I will not attend the Event unless I do not have symptoms of COVID-19 and I am able truthfully and accurately respond to the ODVA’s health screening questionnaire with responses that indicate my attendance does not pose a heightened risk to myself or others. In addition to all other rules relating to my attendance at the Event, I agree to comply with all COVID-related laws, guidelines, protocols, or procedures that may be implemented by ODVA, the federal and California state government, and the venue as applicable at the time of the Event for the duration of my stay, in order to protect as much as possible the health and safety of all attendees, including any directives for frequent hand washing, social distancing, and use of face masks when indoors. I agree that if I experience any symptoms associated with a Communicable Disease during my attendance at the Event, I will inform ODVA staff as soon as reasonably possible and depart the Event.

I understand that ODVA’s health and safety protocols for the Event require proof to be shown at registration of a negative COVID-19 test from a credentialed COVID-19 testing provider within the past 72 hours prior to registration check-in OR of full vaccination against COVID-19 (at least two weeks after final vaccine dose). Full vaccination means that a person has received their primary series of COVID-19 vaccines, as defined by the CDC on www.cdc.gov/coronavirus/2019-ncov/vaccines/stay-up-to-date.html. I understand that these health and safety protocols apply to all attendees, including any guest accompanying me to any ODVA events. People who have recently tested positive (but have been released from isolation) may present a public health release letter or a letter from their doctor dated within 90 days of the event in place of a recent negative test.
