Test Order Form (Part 1 of 2)

The Test Order Form is Part 1 of 2. Once you place your order by submitting this form, you will be redirected to the Test Details Form (Part 2 of 2, PDF), where you can provide the detailed test contact information and product information that ODVA needs prior to your test. The Supplemental Release for Third Party Communication is available as part of the Test Details Form.

Test by Vendor Order Form

Use the Test by Vendor form to order an update of a Declaration of Conformity. 

Test Checklist

Before you order a test, please complete the following items:

1. Ensure that your company has executed a Terms of Usage Agreement and follow the requirements contained therein.  
2. Self-test your device using the relevant Protocol Conformance Test Software Tool available from ODVA. Self-testing is strongly recommended by ODVA.
